Handwritten Application for an Absentee Ballot

You may submit a handwritten application to request an absentee ballot by including the following information in a letter and mailing it to:

  1. Full name
  2. Your date of birth
  3. Home address at which you are registered to vote
  4. (optional) Mailing address if different from your registration address
  5. One of the following items showing proof of your identification:
    1. Ohio driver’s license number (two letters followed by six numbers)
    2. Last four digits of Social Security Number
    3. Copy of a current and valid photo identification or a current (within last 12 months) utility bill, bank statement, or other government document that shows your name and current address
  6. The election for which you are requesting an absentee ballot; If the request is for a partisan primary election ballot, include your political party affiliation
  7. A statement that you are a qualified elector
  8. Sign your name and date the letter
Statement on Unsolicited Applications

We’ve received multiple calls and emails expressing concerns over mailings of absentee application postcards to voters. These do not come from the Geauga County Board of Elections. We have no control over who mails what to voters. We will fulfill any completed applications which comply with the requirements of Ohio Revised Code 3509.03.

If you are unhappy with having received an unsolicited application, we suggest not using it, instead printing an application from our website, or calling us at 440-279-2030 to request an application.

The Geauga County Board of Elections will hold a Special Board Meeting on August 2, 2022, at 6:15 a.m. at the Board offices at 470 Center St. It can also be viewed at the link below.

The meeting is to conduct the August 2, 2022, Primary Election and

  • Open the polls
  • Determine which Ballots may be remade and review over votes, blank ballots, and determine voter intent if necessary
  • Review insufficient absentee ballot envelopes
  • Any other business necessary to legally complete certify the unofficial canvass
  • Discuss employees processing provisional ballot envelopes
  • Consider the date of Official Canvass, public test
  • Certification date
  • Audit date
  • Charles Walder, Geauga County Auditor
  • Executive Session per ORC 121.22(G)(3) to conference with the Board’s legal counsel concerning pending litigation
  • Parkman Petition
  • Director Report
  • Deputy Director Report
  • Close the polls
  • Certify Unofficial Canvass
  • Adjourn>
